Are you a new player?
Download and fill out this registration form if you are not sure where you want to place your player and send the form to my email address i.hockey@jiri-dvorak.com.
I will contact you within two days of sending and we will discuss the details together, such as the first training session and its final evaluation and suggestions for improvement, including a plan for the future.
The form can be completed at the training location. We require at least one parent/legal guardian to complete it.
